Prescriptions
- The prescription can be ordered by calling the registration number – tel. 22 593-14-30 during opening hours of our clinic
- The service is provided only for patients who have submitted declarations of choice of doctor POZ in the Clinic NZOZ SGGW
- A prescription order can only be placed for solid medicines, as recorded in the card
- Please prepare a list of medicines – the registration worker has no insight into the patient’s medical history
- The prescription is issued for a period of not more than 4 months
- The deadline for the implementation of the need for medicines is seven working days. During the holiday period and during the pandemic period, the time limit may be extended
- Your doctor may refuse to list your medicine or write out a different amount of packaging than you ordered
- You do not need to have a paper version of the document when using e-Prescriptions. Simply enter your Social Security number and 4-digit passcode when visiting the pharmacy
- We recommend using an Online Patient Account – https://pacjent.gov.pl/internetowe-konto-pacjenta
- By registering on the portal you will receive a prescription code via SMS
- Please note that if you have ordered more than 5 medicines, you will receive 2 codes in one SMS (the code is assigned to a maximum of 5 drugs)
- Due to the threat of sars-cov-2 coronavirus outbreak, our patients who received an e-prescription during the telecod and do not have an Online Patient Account (IKP) will be informed by telephone about the prescription code
